When/if applicable, NHPHP can provide the following types of written reports:
- Compliance reports at the request of the client or credentialing entity, employer or insurance company.
- Comprehensive evaluation reports to a licensing board per the requirements of a formal order for evaluation by NHPHP.
Important Points to Remember:
- NHPHP must have a valid Release of Information (ROI) for all report recipients.
- Reports typically have a 14-day administrative turn around.
- Please contact NHPHP to discuss requests for reports that fall outside those listed above.
